Certificate Information
Students who enrolled after September 28th, 2008, and require a paper
copy of the "Proof of Beginner Driver Education Course Completion,"
must download the application form and mail it to the MTO. The
address for MTO is provided on the form. The student may also take
this form into their local Vehicle License Office and receive a copy
immediately.
Note: Sometimes the report does not reflect the Driver's Education as
completed, therefore, it is essential to double check the report and
confirm that the Beginner Driver Education was completed, including
the completion date.
Driving schools will continue to issue certificated to students who
enrolled prior to September 28th, 2008. The Driver License History
search may only be requested by the license holder as it contains
personal information that cannot be disclosed to anyone else. A
separate application must be completed and signed by each applicant.
